Projection changes how anatomy appears on a chest radiograph. AP imaging can magnify the heart and mediastinum, poor inspiration can increase basal density, and rotation can create false mediastinal widening. The report should account for these effects before naming disease.
Upright PA and lateral radiographs are better suited to standard assessment. Portable AP imaging is used for patients who cannot travel or are clinically unstable. A single supine image and low inspiration are common in this setting, so standard thresholds carry less certainty.
