Pendiente
  • What is the patient's status?
  • What is the maximal degree of stenosis?
  • Are there any vessels that cannot be evaluated due to motion or other artifacts (i.e. cannot exclude obstructive CAD)?

Qué hace esta calculadora

Combines coronary stenosis severity with plaque and ischemia modifiers for standardized CCTA reporting.

Enter the highest stenosis category and applicable modifiers to assemble the CAD-RADS result.

Antes del cálculo

  • Use a technically adequate coronary CTA and identify whether the clinical setting is stable chest pain or an acute presentation.
  • Assess the highest stenosis, total plaque burden, and any applicable ischemia or stent/graft modifiers.

Alcance clínico

Use CAD-RADS 2.0 for coronary CT angiography in patients with stable or acute chest pain. It communicates the most severe per-patient coronary stenosis together with plaque burden and applicable modifiers.

Aplicabilidad y exclusiones

  • Do not apply CAD-RADS to a non-coronary CT or calcium-score-only examination.
  • The category does not replace the full impression, patient-specific clinical judgment, or separate acute and stable chest-pain management pathways.

Método y lógica de cálculo

The highest luminal stenosis determines CAD-RADS 0-5. Add P1-P4 for overall plaque burden and modifiers such as N, HRP, S, G, E, or an ischemia result when applicable.

Tabla de interpretación

CategoryStenosis summary
0No plaque or stenosis
1-2Minimal to mild stenosis
3Moderate stenosis
4A-4BSevere stenosis or high-risk distribution
5At least one total occlusion
NNon-diagnostic limitation

Errores frecuentes

  • Assigning a definitive category when important coronary segments are not evaluable.
  • Using the stenosis category alone while omitting plaque burden, modifiers, symptoms, or downstream clinical context.
Qué no indica el resultado

CAD-RADS standardizes coronary CTA reporting; it does not replace the full clinical treatment decision.

Ejemplo práctico

Caso: Fictional example: CCTA shows a maximal 50-69% stenosis with moderate overall plaque burden and no additional modifier.

Cálculo: Maximal stenosis selects CAD-RADS 3; moderate plaque burden adds P2.

Interpretación: Example output: CAD-RADS 3/P2. The report must still describe lesion location, extent, symptoms pathway, and patient-specific next steps.

Preguntas sobre esta calculadora

What determines the numeric CAD-RADS category?

The most severe coronary luminal stenosis on a per-patient basis.

What changed in CAD-RADS 2.0?

The 2022 update expanded plaque-burden reporting and incorporated CT-FFR or CT perfusion ischemia modifiers when performed.
