Clinical scope
Use CAD-RADS 2.0 for coronary CT angiography in patients with stable or acute chest pain. It communicates the most severe per-patient coronary stenosis together with plaque burden and applicable modifiers.
Applicability and exclusions
- Do not apply CAD-RADS to a non-coronary CT or calcium-score-only examination.
- The category does not replace the full impression, patient-specific clinical judgment, or separate acute and stable chest-pain management pathways.
Method and calculation logic
The highest luminal stenosis determines CAD-RADS 0-5. Add P1-P4 for overall plaque burden and modifiers such as N, HRP, S, G, E, or an ischemia result when applicable.

Worked example
Case: Fictional example: CCTA shows a maximal 50-69% stenosis with moderate overall plaque burden and no additional modifier.
Calculation: Maximal stenosis selects CAD-RADS 3; moderate plaque burden adds P2.
Interpretation: Example output: CAD-RADS 3/P2. The report must still describe lesion location, extent, symptoms pathway, and patient-specific next steps.
