Antes del cálculo
- Choose the ultrasound or MRI pathway before entering descriptors; the two pathways are not interchangeable.
- Confirm adnexal origin, technical adequacy, symptoms, menopausal status, and prior imaging.
Alcance clínico
Use O-RADS to describe and risk-stratify ovarian or other adnexal lesions on ultrasound or MRI. Ultrasound is the primary pathway; MRI is a complementary problem-solving pathway with separate descriptors and logic.
Aplicabilidad y exclusiones
- Do not apply an ultrasound category from MRI findings, or an MRI category from ultrasound findings.
- Pregnancy, acute symptoms, known malignancy, and lesions clearly arising outside the ovary/adnexa require clinical context beyond the score.

Select the imaging pathway first. The calculator then combines pathway-specific morphology, solid tissue, vascularity or enhancement, and other descriptors into an O-RADS assessment category.

Errores frecuentes
- Mixing ultrasound morphology and Doppler descriptors with MRI enhancement rules.
- Applying the category's management line without accounting for acute symptoms, pregnancy, prior malignancy, or local practice.
The category standardizes imaging risk; it does not determine treatment by itself.
Ejemplo práctico
Caso: Fictional example: a unilocular smooth cystic adnexal lesion is assessed on ultrasound without a solid component.
Cálculo: Choose the US pathway and enter the lesion morphology, size, wall features, and Doppler information shown on the examination.
Interpretación: The generated category must be checked against the complete O-RADS US table, menopausal status, symptoms, and local management policy.
