What this calculator does

Organizes ovarian and adnexal lesion descriptors into the applicable O-RADS assessment pathway.

Choose the imaging pathway and enter the visible lesion descriptors. The tool applies the matching US or MRI decision logic.

Clinical scope

Use O-RADS to describe and risk-stratify ovarian or other adnexal lesions on ultrasound or MRI. Ultrasound is the primary pathway; MRI is a complementary problem-solving pathway with separate descriptors and logic.

Applicability and exclusions

  • Do not apply an ultrasound category from MRI findings, or an MRI category from ultrasound findings.
  • Pregnancy, acute symptoms, known malignancy, and lesions clearly arising outside the ovary/adnexa require clinical context beyond the score.

Method and calculation logic

Select the imaging pathway first. The calculator then combines pathway-specific morphology, solid tissue, vascularity or enhancement, and other descriptors into an O-RADS assessment category.

Worked example

Case: Fictional example: a unilocular smooth cystic adnexal lesion is assessed on ultrasound without a solid component.

Calculation: Choose the US pathway and enter the lesion morphology, size, wall features, and Doppler information shown on the examination.

Interpretation: The generated category must be checked against the complete O-RADS US table, menopausal status, symptoms, and local management policy.
