Parenchymal (brainstem-dominant)
- ▶ Brainstem > thalamus > white matter
- ▶ T2/FLAIR high signal; +/- enhancement if acute
- ▶ Chronic: atrophy, T1 black holes
- Mimics: MS / ADEM / tumor / abscess
