What Is a Complete Abdominal Ultrasound? Organs, Preparation, and Limits

What does "complete abdominal ultrasound" mean?
A complete abdominal ultrasound uses sound waves to assess organs and structures in the abdomen in real time. It does not use ionizing radiation. The exact scope depends on the clinical question, institutional protocol, and patient factors, so the label does not guarantee an identical organ list in every imaging practice.
In adults, the core examination commonly includes the liver, gallbladder and bile ducts, pancreas, spleen, both kidneys, the abdominal aorta, and visible retroperitoneal structures. Whether the urinary bladder and female or male pelvic organs are included depends on the requested examination and local protocol.
Short answer: A complete abdominal ultrasound commonly assesses the liver, gallbladder, bile ducts, pancreas, spleen, kidneys, abdominal aorta, and the abdomen for free fluid. The bladder and pelvic organs may require additional or dedicated imaging.
Which organs are assessed?
Liver
The examination assesses liver size, contour, parenchymal echogenicity, and homogeneity. It may evaluate focal lesions, bile duct dilatation and, when relevant, the portal and hepatic veins. Ultrasound findings are interpreted with the clinical history, laboratory results, and prior imaging.
Gallbladder and bile ducts
The gallbladder is reviewed for distention, wall appearance, stones, sludge, and surrounding changes. The intrahepatic and extrahepatic bile ducts are assessed in clinical context. Eating before the examination can contract the gallbladder, which is why preparation may matter.
Pancreas
The head, body, and tail are assessed when visible. Bowel gas often obscures part of the gland, especially the tail. A segment that cannot be seen should be documented as a limitation rather than described as normal.
Spleen
The spleen is assessed for size, contour, and parenchymal appearance. Enlargement, focal abnormalities, and surrounding fluid are reported when relevant.
Kidneys
Both kidneys are assessed for size, contour, cortical thickness and echogenicity, collecting-system dilatation, conspicuous stones, and focal lesions. Ultrasound may not show every small or ureteral stone.
Abdominal aorta and retroperitoneum
Visible segments of the abdominal aorta and selected retroperitoneal structures may be assessed. A dedicated protocol may be needed for aneurysm screening, detailed vascular measurements, or a specific retroperitoneal question.
Urinary bladder and pelvic organs
The bladder may require adequate filling. Inclusion of the uterus and ovaries or prostate varies by practice and examination request. A detailed pelvic assessment may require dedicated transabdominal, transvaginal, or other targeted imaging.
Free intraperitoneal fluid
Ultrasound can detect free fluid in the peritoneal cavity. Trauma, ascites, or a suspected localized collection may require a focused protocol rather than a routine complete abdominal study.
How does an upper abdominal ultrasound differ?
An upper abdominal ultrasound usually concentrates on the liver, gallbladder, bile ducts, pancreas, spleen, kidneys, and upper abdominal vessels. A "complete abdomen" request may extend to the bladder, lower abdomen, or a basic pelvic assessment.
This terminology is not perfectly standardized across institutions or countries. The safest interpretation is the scope defined by the imaging center and the clinical question on the request. A focused ultrasound may be more appropriate than a general examination when one organ or symptom is the primary concern.
How should a patient prepare?
Preparation varies with examination scope, and the imaging center's instructions take priority.
- Fasting for a specified period may be requested for gallbladder and upper-abdominal assessment.
- Drinking water and keeping the bladder full may be requested when the bladder or pelvis will be assessed.
- Instructions about regular medicines should come from the referring clinician or imaging department.
- Prior ultrasound, CT, or MRI reports may be useful for comparison.
- Diabetes, pregnancy, recent procedures, and dietary requirements can change preparation instructions.
Generic online advice should not replace instructions from the facility performing the examination. Follow the center's specific fasting interval and fluid instructions.
What happens during the examination?
The patient usually lies on an examination table while gel is applied to the abdomen and a transducer is moved over different regions. The sonographer may ask the patient to hold a breath, breathe deeply, or turn to improve visualization. Doppler ultrasound may be added when blood flow needs assessment.
Standard diagnostic ultrasound does not use ionizing radiation. It is usually painless, although pressure over a tender area can cause temporary discomfort.
What are the limitations?
Ultrasound depends on the operator, equipment, body habitus, and available acoustic windows. Bowel gas can block sound transmission, and deeper structures or portions of an organ may not be adequately seen. Inadequate fasting may limit gallbladder assessment, while poor bladder filling may limit lower abdominal and pelvic assessment.
Failure to visualize a structure does not exclude disease. If the clinical question remains unanswered, a targeted ultrasound, CT, MRI, or another test may be recommended based on the patient's symptoms, laboratory results, and clinician assessment.
How should the report be read?
The Findings section describes the organs assessed and any technical limitations. The Impression summarizes the clinically important findings and, when appropriate, follow-up recommendations. "Not visualized" or "limited assessment" describes examination scope; it is not the same as a normal finding.
