LI-RADS Calculator for CT, MRI, US and CEUS

LI-RADS calculator covering CT/MRI diagnostic categories, ultrasound surveillance, CEUS, and treatment response assessment pathways.

Inputs

Encodes the main CT/MRI diagnostic table, US surveillance management, CEUS APHE/washout table, and v2024 TRA response categories. It still expects the radiologist to identify LR-M, TIV, benign entities, and eligible LI-RADS population correctly.

Result

Complete the required fields to calculate a result.

What this calculator does

Structures liver observation categorization across the supported LI-RADS imaging pathways.

Select the examination pathway and supply its major features, size, and ancillary pathway inputs.

Clinical scope

Use the appropriate LI-RADS pathway for patients who meet that pathway's at-risk population criteria: CT/MRI diagnosis, ultrasound surveillance, CEUS diagnosis, or treatment response. Each pathway has distinct eligibility and category rules.

Applicability and exclusions

  • Do not apply diagnostic CT/MRI or CEUS categories when the patient does not meet the defined at-risk population criteria.
  • Do not mix surveillance, diagnostic, and treatment-response descriptors; pediatric patients and vascular or benign mimics may require separate frameworks.

Method and calculation logic

Select the pathway before entering features. CT/MRI uses major and ancillary features; surveillance US uses observation and visualization findings; CEUS uses enhancement and washout; treatment response uses therapy-specific viability criteria.

Interpretation table

PathwayRepresentative categories
CT/MRI diagnosticLR-1 to LR-5, LR-M, LR-TIV, LR-NC
US surveillanceUS-1 to US-3 plus visualization score
CEUS diagnosticCEUS LR categories
Treatment responseTherapy-specific LR-TR categories

Worked example

Case: Fictional example: an observation in an eligible at-risk patient is evaluated on multiphasic MRI using the CT/MRI diagnostic pathway.

Calculation: Enter size, nonrim arterial phase hyperenhancement, major features, and any ancillary-category adjustment only after confirming pathway eligibility.

Interpretation: Check the generated LR category against the pathway's current diagnostic table and report all relevant observations, not only the highest category.

How to use this calculator

  1. Complete the visible imaging and measurement fields. Required inputs are marked in the workbench.
  2. Review the calculated category, score, measurements, and any recommendation or warning.
  3. Copy the report-ready text only after checking the result against the complete examination and source guideline.

Calculator-specific questions

Can LI-RADS be used in every patient with a liver lesion?

No. Diagnostic pathways are limited to specifically defined at-risk populations.

Are CT/MRI, CEUS, US surveillance, and treatment response one scoring system?

No. They are related but distinct algorithms; always select and cite the correct pathway and version.

Are patient measurements sent to RadPhrases?

No. Calculator inputs and results are processed in the browser and are not submitted by this page.
